EXAMS FOR FAA MEDICAL FLIGHT CERTIFICATION
At MyMD Personal Medicine in Bend, Oregon, we offer 1st, 2nd and 3rd class physicals for FAA Flight Certification, meeting government regulations. Dr. MacDonell, a senior AME, performs a basic physical examination, including all items required by the FAA for completion of certification application forms. We will submit the forms to the FAA headquarters for approval.

To complete the examination, the doctor will verify a patient’s height and weight and a urine sample is required. The doctor will test color vision, blood pressure and pulse, as well as other tests as required by the FAA, including an EKG for 1st Class.
For the FAA examination, the 1st Class Certificate will require a minimum 45 minutes to 1 hour. 2nd and 3rdclass will take approximately 30 – 45 minutes.
GOVERNMENT FORMS AND EXAM REQUIREMENTS
The FAA MedXPress system allows anyone requiring an FAA Medical Certificate or Student Pilot Medical Certificate to electronically complete the FAA Form 8500-8 online. Information entered into MedXPress will be transmitted to the FAA and will be available for the doctor to review at the time of your medical examination.

Complete the online form above. If you answer “yes” to any of the questions, please go to the FAA website, http://www.faa.gov/ to further research the type in the condition and thoroughly understand your requirements. (Example…type in hypertension and you will be given a list of information that you need to bring with you to your doctor’s appointment when you have your physical examination). IF YOU DO NOT have the correct documentation or information about your specific conditions, it will delay your application process.
If glasses or contacts are worn they need to accompany you to your exam so that eyesight may be checked with and without the optical aids. IF YOU DO NOT bring your glasses with you, it will delay your application process.
IMPORTANT NOTE: If the airman needs to return to see the physician due to additional documentation or other requirements, there will be an additional charge for the return visit. The doctor will charge for each visit required to complete the application process.
